The Champlain Regional Planning Table (RPT) is seeking proposals from qualified consultants/firms to assist with our capacity to meet our stated objectives to our funder, the Champlain Local Health Integration Network (LHIN), and to the community that the RPT represents. The focus of this RFP is primarily in the realm of system planning and related administrative support/project management needed by RPT to carry out its function. The RPT currently receives funding from the LHIN with Family Service Ottawa being the agency who manages the funds. The successful candidate will however report to the chairs of the RPT. The full report to the LHIN is Appendix B. The RPT has confirmed funding from the LHIN until March 31, 2020.

We are in search of a consultant with subject area expertise in the areas of capacity building and strategic operations to assist members of the network in maximizing our individual and collective impact. This project will work with network members to coordinate a common assessment of our collective strengths, our current primary care capacity pressures, and areas for continued strategic leadership. Based on the assessment the consultant will develop concrete and realistic recommendations that support a coordinated, efficient, and effective approach to primary care capacity building while considering the influences of organizational culture and the unique reality of rural and Francophone serving organizations.

Community Programs Support Worker – Summer Student

Seaway Valley Community Health Centre is seeking applications for a Community Programs Support Worker (CPSW) – Summer Student, at 30 hours per week for an 8 week period commencing June 25, 2018.

The CPSW will work as a member of an inter-professional team to provide practical hands-on assistance and support to a variety of programs and services in the Centre and out in the community. The CPSW must have a strong interest in supporting the health and wellbeing of local people working through a team and partnership approach and be comfortable working with vulnerable populations.
