The Cardiac Rehab program is designed to offer support and guidance to the residents of Cornwall and surrounding area who are living with heart disease. We work in partnership with the University of Ottawa Heart Institute and the Ontario Telemedicine Network to provide the specialized rehabilitation care our clients require to succeed. Our Registered Nurse and Physiotherapist work collaboratively with our clients to create individualized treatment plans and empower those individuals with the knowledge and resources they need to live a healthier life.
Our clients include those who have had recent cardiac events or procedures such as:
- Myocardial infarction (heart attack)
- Coronary artery bypass graft
- Angioplasty (stent)
- Valve repair
- Atrial fibrillation and other arrhythmias
- Coronary artery stenosis
- Spontaneous coronary artery dissection (SCAD)
- Heart transplant
Some of the services we offer include:
- Cardiovascular risk assessment
- Assessment of cardiovascular response to exercise
- Education on home exercise program
- Review and education regarding cardiac medication
- Education on self-management and self-monitoring
- Group exercise classes
- Regular follow up with a health care Provider
- Referral to stress management
- Referral to dietitian for heart healthy nutrition
- Referral to lung health and support for smoking cessation
