Community Programs Supervisor – Full-Time Position

Responsibilities: As a member of the inter-professional team, the Supervisor of Community Programs is responsible for oversight of day-to-day operational issues, planning and coordination of the Community Programs Team. The Supervisor reports directly to the Manager of Primary Health Care and provides support on quality improvement initiatives.  The Supervisor must have excellent knowledge of health promotion principles and supervisory experience in a health care or health care related environment. Working collaboratively with Management Team, the Supervisor supports effective team functioning and develops and maintains strong interpersonal relationships internally and externally with community partners.  The Supervisor provides on-going orientation and supervision to the Community Programs Team related to SVCHC’s policies, procedures and protocols and supports the Team on chronic disease program planning, scheduling and delivery matters.  Refer to our Organizational Chart for a better understanding of the Community Program Team/programs. The Supervisor provides consultation and education and facilitates and supports personal development groups and community initiatives related to health promotion and illness prevention. As SVCHC provides services across Stormont, Dundas and Glengarry Counties, outreach is a required element of this position.

Qualifications:An Allied Health Baccalaureate degree from a recognized university or equivalent experience, along with three to five years’ experience in community health or related field. Current registrations with the respective Ontario College and professional associations as relevant. Excellent understanding and appreciation of determinants of health and integrating this into service delivery. Demonstrated ability to work effectively in an inter-professional team environment. Proficiency in working in a computerized environment and experience working with electronic client record is required. Assessment skills, motivational interviewing, individual counselling, group development and facilitation and care plan development considered strong assets.

Community Programs Supervisor  – Job Description

Registered Dietitian – One-Year Contract

Responsibilities: As a member of the inter-professional team, the Dietitian plans and directs the nutritional care of Centre clients and participates in community health education (groups and individuals) to promote better nutrition. The Dietitian has excellent knowledge in health promotion and in the process of behavioural change. Demonstrates innovative approaches aimed at motivating and engaging clients. The Dietitian provides assessment of client nutritional status and develops individualized nutritional plans, provides nutritional counselling and relevant education. Develops and maintains formal and informal networks within the community and provides consultation and education to the SVCHC team.  Develops, facilitates and supports personal development groups and community initiatives related to nutrition. We provide service across Stormont, Dundas & Glengarry so outreach is a necessary responsibility of this position.

Qualifications: Baccalaureate degree from a recognized university in foods and nutrition with two or more years experience in counseling and community nutrition. Completion of an internship accredited by the Partnership for Dietetics Education and Practice (PDEP). Current registrations with the College of Dietitians of Ontario and professional associations as relevant. Skilled in clinical assessment, motivational interviewing, individual counselling, group development and facilitation and care plan development.  Excellent understanding and appreciation of determinants of health and integrating this into service delivery. Demonstrated ability to work effectively in an inter-professional team environment. Nurse Practitioner – Locum

Primary Care Nurse Practitioner
Locum Position, Leave Replacement up to 28 weeks
Cornwall and Area

SVCHC is recruiting a Locum Nurse Practitioner for a leave replacement for up to 28 weeks commencing December 2nd, 2019 (or as soon as possible). We offer a competitive salary of $ 66.14 per hour, (which includes 4% in lieu of vacation), plus HOOPP. This position involves working 35 hours week, inclusive of one evening per week, Monday through Friday. Successful candidates must undergo a vulnerable sector screening to the satisfaction of SVCHC.

POSITION JOB DESCRIPTION/RESPONSIBILITIES:
The Primary Care Nurse Practitioner (NP) provides a wide range of primary care services including comprehensive triage, screening, physical assessment, diagnosis and treatment as well as counselling, interpretation of test results, referral and follow-up, health education and preventative care to clients with varied health, social, cultural and economic needs. The NP is responsible for establishing and maintaining strong communication pathways between clients, primary care providers, specialists, our local hospitals and other community resources. The NP encourages individuals to take responsibility for their own health by involving them in risk factor and health problem identification, goal setting and the choices of interventions for disease treatment and prevention and health promotion. The NP is responsible for establishing and maintaining a close work relationship and communication pathways between clients, primary care providers, specialists, our local hospitals and other community resources. The NP must possess the ability to function independently and as a member of an inter-professional team in a culturally sensitive manner.

EDUCATION, SKILLS & QUALIFICATIONS:
• Baccalaureate in Nursing from a recognized university.
• Current and valid registration in the Extended Class with the RNEC College of Nurses of Ontario.
• 3-5 years of experience is preferred.
• Previous critical care and emergency nursing experience an asset
• Demonstrate effective time-management skills and strong decision-making skills
• Must possess strong organizational, communication and interpersonal skills
• Proficiency working in a computerized environment and working with electronic client records
• Previous experience working with culturally diverse and low-income populations.
• Experience and understanding of rural communities and rural health needs.
• Excellent communication skills in English (oral and written) required; French and other languages considered an asset.
• Valid driver’s license and access to a vehicle required.

The Seaway Valley Community Health Centre is recruiting for a resourceful, tech savvy and highly organized individual to fill the position of Program Assistant for a one-year contract in Cornwall.

Key Responsibilities:

  • Coordinates intake process for group programs.
  • Provides support for group program Staff, Management Team and the Executive Director
  • Responsible for administering a new group program software from Blue Lemon.
  • Coordinates media advertising, including social media (Facebook/ Twitter)
  • Coordinates information packages and logistics for various events, workshops, and meetings.
  • Proficient in use of Word, Publisher, PowerPoint, PDF to create updates promotional posters, invitations, and handouts for events and workshops
  • Works well under pressure with short deadlines.
  • Purchases supplies as needed by the health centre.

Qualifications:  Post-secondary Secretarial diploma, office or business administration diploma or equivalent combination of education and experience. Minimum of 3 years experience as administrative support in a clinical or human service organization. Experience in dealing with the public in a mature and helpful manner. Ability to work in a fast-paced environment and manage a heavy workload.  Advanced computer proficiency is required. Proficiency in English is required and the ability to communicate in French is an asset.

Primary Care Assistant – Summer Student

Seaway Valley Community Health Centre is seeking applications for a Primary Care Assistant (PCA) – Summer Student, at 30 hours per week for an 8-week period commencing July 8, 2019.
As a member of an inter-disciplinary team the PCA will help monitor client flow, maintain examination rooms and supplies and performs other related tasks to ensure the smooth running of the centres primary care services.
