Seaway Valley Community Health Centre is conducting its annual Client Satisfaction Survey.  Those who have come in-person, may have completed a survey already.  You only need to complete the survey once.

This year, Seaway Valley Community Health Centre is also emailing its clients to ask them to complete the survey. While the email clients provided is intended for health and wellness needs, given the current COVID-19 pandemic, we are unable to complete our annual Client Satisfaction Survey in the usual in-person format for many of our clients.  We are hoping you are willing to complete the survey on the link below.

Survey results are anonymous and confidential and help us to better serve you. As an incentive for participation, your name can be entered in a draw for a $25 grocery gift card upon completion.
