Children between the ages of 0 to 5 who do not have a doctor or nurse practitioner.

Is there a cost to the program?

This is a free service that is extended to all children that qualify.

Does my child need a health card to register for the program?

No. Your child doesn’t need a health card to be part of the program. 

I am a newcomer to Canada. Can my child access Healthy Sprouts?

Absolutely. All are welcome, including asylum seekers.

My child already has a doctor or nurse practitioner but I would like to switch to the Healthy Sprouts program. Can I?

Sorry, no. The program is only for children who don’t currently have a healthcare practitioner. 

If I am involved with this program, should I stop looking for a doctor or nurse practitioner?

No. You should actively continue the search for a doctor or nurse practitioner for your child. Healthy Sprouts is designed to offer limited, temporary care until you can find your own provider (or until your child reaches the age of 5). 

We encourage everyone to reach out to local doctors and nurse practitioners who may be accepting new clients.

Can I bring my sick child to the clinic?

This program is only for your child’s regular wellness checks only. If you already have an appointment, please call us at 613-935-4393 to reschedule.

If your child is sick, click HERE to find out where you can take them for care.

Since I’m bringing my toddler, can the nurse also provide care for my older child or myself?

Unfortunately, we can only see children under the age of 6. 

For other family members, reach out directly to local doctors and nurse practitioners.

Does the program offer prenatal care?

Not at this time. Please contact the Eastern Ontario Health Unit for more information.

I'm a health professional, do you have a referral form?

Yes.  Please complete the following form and fax it to 613-936-0521. 

Health care professionals will also soon be able to refer to us directly through Oceans.
