❤️ Heart Health Screening Program

Heart disease often develops silently—and for many in our community, it goes undetected.

Our Heart Health Screening Program is designed to bridge a critical gap by reaching individuals who are not currently connected to a family doctor or nurse practitioner, or who haven’t had recent health screenings. In Cornwall and the surrounding area, many residents are missing out on routine cardiovascular screening—putting them at risk for undiagnosed and unmanaged conditions.

Through accessible, community-based clinics, we bring screening directly to where people are. These screenings help identify risk factors early and ensure individuals receive timely support. Those found to be at risk can be fast-tracked to appropriate care, including referrals to specialists, while being supported by our interdisciplinary team as they work toward connecting with a primary care provider.

This initiative began through a valuable partnership with the University of Ottawa Heart Institute and their One Mission: Canadian Hearts initiative.  While this collaboration has allowed us to begin this important work, thanks to the support of community sponsors, the program will help us reach even more people—especially in underserved and off-site community locations.

By focusing on early detection and prevention, this program not only improves individual health outcomes but also helps reduce long-term strain on our healthcare system.
