Primary Care Update

We are currently taking individuals off of Health Care Connect to be rostered to a doctor or nurse practitioner.

If you do not have a family doctor or nurse practitioner, please register with Health Care Connect to be considered for primary care attachment.

Please note that we cannot accept individuals who already have a primary care provider (MD or NP).

SVCHC Waitlist
As of November 2025, SVCHC has closed its waitlist.

If you previously completed and submitted a waitlist form to Seaway Valley Community Health Centre, please note that your information is already on file.

Our team is currently contacting individuals who registered with Health Care Connect in the order their registration forms were received.

As we move through the list, someone from SVCHC will reach out to you directly when it is your turn to begin the onboarding process
